Lilith and Inarius gathered like-minded followers and retreated to Sanctuary for a break from the Eternal Conflict Diablo IV Gold, and eventually, the demon and the rogue angel had Rathma, a first-generation nephalem, and the first child to be conceived between two such beings. Others followed, but the rogue angels and demons began to fear their offspring's strength increasing and were frightened by the new species of beings. They wanted to kill them and Lilith in fear of her children and seeing them as the key to ending conflict for good was determined to kill every follower of Inarius who sought to eliminate them.

Inarius was appalled at Lilith's savagery and betrayal, and although he was unable to get himself to commit suicide and he was able to banish her in the Void. He also made sure that every subsequent generation of nephalem was weaker in order to avoid the problems which the demons who escaped and angels were afraid of their offspring rebelling against them. However, Lilith was still revered in mythology, and was referred to as the First Mother by some , and inciting cults to worship her. Lilith made a return to Sanctuary previously and altered the ability of the Worldstone to grant nephalem abilities again. Even though Lilith was able to inflict chaos and return to Sanctuary in various forms, Inarius was successfully able to restore her exile to the Void.

In the official announcement trailer in the trailer for Diablo 4. a member of the Triune managed to trick two adventurers and one scholar into entering a ritual chamber to ensure that Lilith could finally be released thousands of years following her initial exile from Sanctuary. Lilith's return inspired a new group of worshipers and cultists across the world, since her return was viewed by some as the way to resolving an Eternal Conflict. Lilith brings out the most dark urges within the souls of all that encounter her, and the return of Lilith to Sanctuary is likely to bring about a return to despair and darkness as well. Her ultimate cheap Diablo 4 Gold goal - to ensure that the nephalem is raised from the dead - is likely to become the primary focus of Diablo 4's story.
